Trauma

MAXILLOFACIAL TRAUMA

Injuries to the face , by their very nature, impart a high degree of emotional, as well as physical trauma to patients. There are a number of possible causes of facial trauma. Motor vehicle accidents, accidental falls, sport injuries, interpersonal violence and work related injuries account for many.

Types of facial injuries can range from injuries of teeth to extremely severe injuries of the skin and bones of the  face. They are classified as

  • Soft tissue injuries ( skin and gums)
  • Bony injuries ( fractures)
  • Injuries to special regions ( such as eyes, facial nerve)

Soft tissue injuries such as, lacerations on face, can be repaired by suturing. Major tissue loss requires either skin grafting or flap surgeries.Fractures of bone of the face are treated by open reduction and internal fixation with help of mini plates.

BURN

ACUTE BURN CARE

Ideal treatment of acute burns is important for early recovery and to prevent post burn deformity. Treatment depends upon depth of burns. In superficial burns, conservative treatment in form of dressings is done. Variety of dressings are available like collagen dressing, silver dressings which help in speedy recovery, decreases risk of infection and need of daily dressing change. They make patients comfortable by reducing pain and shortens hospital stay.

In deep burns, surgery in the form of early excision and grafting is done. If deep burn is left for dressings only, then patient develop infection in wound which causes septicemia and risks patient life. If burn wounds are left open it causes protein loss from wounds which leads to debilitated state and increases morbidity. It also leads to formation of post burn deformities. Early surgery leads to better outcome as it covers the wound, decreases losses from wound, lessens the chances of infection and early recovery of patients. In this procedure , dead layers of skin are removed with help of special instruments and covered with patients own healthy skin.

POST BURN DEFORMITY/CONTRACTURE

Deep burns lead to scarring resulting in post burn deformity. Deep Burn wounds across joint if not managed properly causes post burn contracture.

Scars can be treated with the help of silicon gel sheet, massage and pressure garment , Injection Kenacort within a year of injury. Post burn contracture are treated surgically in form of release of contracture and use of splints.

High cosmetic results are achieved with help of tissue expanders which replaces scarred tissue with normal adjacent skin.

Hand Surgeries
Hand surgery is a specialized branch of plastic surgery which deals with hand injuries, hand deformities. Common hand conditions which impair functions and can be surgically treated are :
  • Carpal tunnel syndrome
  • Dupuytren’s contracture
  • Trigger finger
  • Ganglion removal
  • Rheumatoid arthritis
  • Nerve Palsy
Hand injuries are most common injuries which can be due to road traffic accidents, industrial accidents, accidental or suicidal cuts. Various conditions seen are:
  • Tendon injuries
  • Nerve injuries
  • Finger tip injury
  • Amputation of fingers and hand
  • Avulsion injuries
  • Soft tissue loss
All of these can be treated. Tendon and Nerve injuries can be sutured directly . Finger tip injury can either be treated by skin graft or by flap surgery to restore length of finger. In case of Amputated finger or hand, reimplantation is done to restore form and function. Congenital hand anomalies- They are present since birth. Common anomalies seen are:
  • Syndactyly of fingers
  • Constriction ring syndrome
  • Extra fingers or thumb
  • Absent thumb
